What we collect

Information submitted through the website may include your name, organisation, email address, message content and uploaded supplier invoice files.

What not to upload

Please do not upload patient-identifiable information, clinical records, prescriptions, referrals, test results or any document that is not a supplier/consumables invoice.

How it is used

Submitted information is used to assess free-report applications, produce invoice reports and contact you about your submission. Uploaded supplier files are used for the requested DBX ONE report and should not contain patient-identifiable information.
